| This is a one-year intensive program designed to work with any severity
level of asthma. We can make a difference in the self management and control of asthma; helping to decrease unnecessary visits to the
clinic, urgent care, emergency room, or hospital, along with having fewer days missed at work and/or school. The program includes seven
visits totaling four hours at the Education Center one-to-one with a Certified Asthma Educator who is community focused and will work
hand in hand with health care providers, families, caregivers, schools, and hospitals. An additional three hours are provided for making
home, school and/or hospital visits as needed. |

| Asthma is a chronic inflammatory disease of
the airways. When a person has asthma their airways swell and fill with mucus, and the muscles around the airways
tighten and become twitchy. This causes narrowing of the airways, making breathing much more difficult! |
